Before your first visit, please complete the following:- Fill out the Contact Info section- Fill out the Health History section (don't forget the family history and medication sections)- Review and sign the Financial Agreement (in the Forms and Records > Consent Forms section)- Review and sign the Informed Consent & Bill of Rights (also in the Consent forms section)- Answer the questions in the Depression Screening form (also in the Consent Forms section)You can also use the portal for:- Viewing upcoming appointments- Sending documents to your midwives, office staff, or billing team- Sending messages to your midwives, office staff, or billing team- Looking up information about pregnancy (in the Forms and Records > Educational Forms section)- Viewing the glossary of terms about pregnancy (also in the Educational Forms section)- Viewing and blood work or ultrasound reports related to your pregnancy (in the Forms and Records > Medical Records section)
You can always contact the midwife on call by calling the office number and selecting the option to speak to a midwife right away. Our office number is 425-678-9070.You can also send a secure message through the portal. We respond to messages within 24-72 hours (births may affect our availability to check messages) If you ever have an urgent concern or think you're in labor, DO NOT message us - CALL us! We'd love to hear from you.
We are trying to minimize potential viral exposure. We are offering a combination of in person and virtual visits throughout your care. We also encourage you keep us updated if you feel like you may be coming down with something. If you have symptoms of fever, runny nose, or cough, loss of taste/smell, body aches or other viral symptoms, please call us to help decide whether you can delay your visit by one week or switch to a virtual visit.
