Please make sure you go through all of the tabs and fill in your information prior to your first visit. The last tab, Medications, contains a submit button that you will have to click in order to transmit the information to us. There will also be a list of forms to be completed on the Forms and Records tab. Please fill in all of these! The records release is only needed if you are transferring care from another provider.
